By looking at pic doesn’t look like mid jacking is an option. I fact any sort of jacking it up doesn’t seem possible. Looks like it will have to be taken down and rebuilt on a new subsurface today prevent it from happening again. But I’m not an expert.

The base material was done incorrectly. You can’t just jack it up. It needs to be taken down and redone.
